OUR MISSION

Our mission is to provide comprehensive urgent and chronic primary care to our surrounding communities. Our goal is to provide a hybrid model of easy and affordable access to care, utilizing technology in our forefront, to provide a personal experience with providers you know and trust. We are breaking down the barriers in healthcare that prevent patients from being able to put their health as a priority.

OUR VISION

As primary care providers we believe there has been a gap in healthcare that has allowed many patients to not make their health a priority. With this hybrid model of healthcare we hope to expand chronic care to those that struggle to get to appointments due to health, monetary or scheduling barriers. Our providers understand the need to fill this gap and have seen the benefits of telemedicine providing this care where needed. Telemedicine being the focus of the practice, while understanding at times, it will be necessary to see someone in person. Telemedicine is the way of the future, and we are here to make that transition as easy as possible to fit into peoples lifestyle, while knowing they still have a provider that’s closer to home.

OUR PROVIDERS

Brooke Boyd, FNP-C

Brooke has been a Certified Family Nurse Practitioner since 2018. She received her Associate of Science in Nursing from Kent State University in 2010. She then went on to complete her Bachelor in Science of Nursing from Capella University in 2015, and finished with her Masters of Science in Nursing from Walden University in 2018. Prior to becoming a nurse practitioner, Brooke worked as a registered nurse on labor/delivery, nursery, medical surgical, post-op, and telemetry units. As a nurse practitioner, Brooke has experience in primary care within all age groups from birth to end of life.

Kaley Day, FNP-BC

Kaley has been a Certified Family Nurse Practitioner since 2018. She received a Bachelor of Science in Nursing in 2011 and then her Masters of Science in Nursing in 2018, both from Youngstown State University. Prior to her nurse practitioner career, Kaley worked as a registered nurse on a telemetry unit in a local hospital. As a nurse practitioner, Kaley has experience in primary care within all age groups.
